Program Description
Achieving optimal diabetes management
and individualized glycemic goals often requires insulin therapy. Insulin is
recommended by clinical guidelines to prevent microvascular and macrovascular
complications in people living with type 2 diabetes (T2D) when glycemic levels
remain above the recommended target. However, the significant personal and
logistical burden of insulin regimens, which demand multiple daily injections,
can severely impact a patient's daily life and overall well-being. This
challenge frequently contributes to therapeutic and clinical inertia, where
necessary treatment adjustments are delayed due to concerns over regimen
complexity or patient difficulties. This session is specifically designed for
practicing pharmacists to address these challenges by comprehensively reviewing
the evolving landscape of insulin care, differentiating the efficacy and safety
profiles of existing formulations, and examining the potential impact of
emerging insulin formulations as well as other strategies to simplify care and
alleviate the burden. Presented in a unique, highly interactive workshop
format, this session includes an expert faculty panel that will present a
collaborative patient case and focused discussions. Pharmacists will gain the
necessary skills to identify patients experiencing adherence challenges to
complex regimens, apply best practices in sensitive titration and counseling,
and highlight shared decision-making strategies to overcome inertia and step
into a stronger, more patient-centric leadership role in diabetes care.

Educational Objectives
At the completion of this activity, participants will be able to:
- Identify suboptimal medication use in people with type 2 diabetes (T2D) receiving insulin therapy by evaluating individualized glycemic targets and patient-specific factors
- Use glycemic data to propose guideline-aligned insulin modification plans and facilitate therapy adjustment for a person living with T2D receiving insulin therapy
- Employ motivational interviewing and communication techniques to uncover specific patient-related barriers to insulin use supported by individualized patient action plans
