Program Description
Hypertension greatly increases the burden of stroke and other cardiovascular comorbidities, with approximately 60% of strokes attributable to poor blood pressure control. Traditional management is often hindered by therapeutic inertia and the significant burden of multi-pill regimens, which severely increases dosing complexity, reduces patient adherence, and delays the time to optimal blood pressure control. As new 2025 AHA/ACC guidelines underscore the need for early and aggressive combination therapy, this webinar provides pharmacists with a review of best practices to transition from traditional approaches to optimized single-pill combination (SPC) strategies. Patient selection criteria for escalating treatment from dual SPCs to the novel fixed-dose triple therapy with telmisartan, amlodipine, and indapamide will be examined, utilizing risk stratification tools and integrating superior efficacy data from phase 3 trials. This webinar will equip practicing pharmacists to identify patients with hypertension who might benefit from triple combination therapy initiation, focusing on those with specific comorbidities and elevated risk. Presented in conjunction with the National Community Pharmacists Association, the webinar will specifically equip practical skills for community and independent pharmacists to implement collaborative care, including patient counseling points on adherence, adverse effect management, and titration timelines for this advanced regimen, to seamlessly integrate these evidence-based protocols into their daily practice workflow and significantly improve blood pressure control and treatment adherence in high-risk patients.

Educational Objectives
At the completion of this activity, participants will be able to:
- Identify patients with hypertension with elevated cardiovascular risk who would benefit from early combination therapy based on blood pressure thresholds and comorbidities
- Compare the efficacy, safety, and dosing of triple versus dual antihypertensive combination products using recent trial data
- Apply pharmacist-led counseling and titration protocols to improve initiation, adherence, and persistence with fixed-dose combinations
